Partnering with families of youth who are at risk of out of home placement and who also have a mental health diagnosis, LMCS-WINGA provides support and coordination based upon the family’s and youth’s needs. They work with collaboratively with schools, community supports, and to identify and utilize natural supports for the families. The child and family team consist of a Care Coordinator and Certified Peer Support –Parent and/or Specialist working on behalf of LMCS-WINGA and each individual family to reach goals they identify through a Child and Family Team meeting held each month throughout the process.
