Community Residential Rehabilitation (CRR)
Bridge Health has begun recruiting for our Community Residential Rehabilitation (CRR) home! This home will offer an environment where those who are transitioning from an inpatient environment back to a community setting can learn new skills and have a supportive transition to independent living. The CRR Home will operate 24/7/365 and will house no more than 4 individuals at a time with the goal of transitioning to independent living.
We are hiring for full-time and part-time direct care staff. We will have first, second, and third shifts available Monday–Friday and 12 hour weekend shifts are also available.
Case Manager (TREK)
Bridge Health TREK Foster program is seeking a qualified candidate for the position of Case Manager. Individual selected will supervise the Foster Homes agency and maintain a caseload of foster children. Candidate will be responsible for ensuring adherence to all state, local, and federal guidelines, Bridge Health agency guidelines, and maintaining documentation. Travel is a vital part of the position and will be required. The primary work schedule will be Monday through Friday. Ability to work a flexible schedule and be available for monthly on-call rotation is a must.

Customer Service Representative (Part-time)
Bridge Health is seeking a Customer Service Representative, to work in our IDD program. Under general supervision, assist families with completing Department of Behavior Health and Developmental Disabilities (DBHDD) applications for services, complete payment vouchers, and work with the agency business office to arrange for payment/funding of services. Maintain good communication with families as services are arranged until they are in place successfully. Maintain spreadsheets to ensure that funds are being utilized to their full potential. Collect information necessary to ensure enrollment into the Electronic Medical Record (EMR) system. Prepare admission and discharge documentation and use reports to ensure documentation is accurate. Good communication skills and good customer service necessary to work with internal and external stakeholders are a must for the person selected to fill this role. This position is part-time however, the person selected for this position would be eligible to receive four hours of PAID HOLIDAY leave for agency Core Holidays, defined as Holidays for which the agency is closed for business, and the person selected will also be included in agency morale incentives quarterly.

Case Manager
Bridge Health is looking to hire a Case Manager for our Adult Case Management program. This individual will provide education and support to individuals utilizing community-based resources. The Case Manager will assist in developing goals that address various needs, including mental health, medical issues, substance abuse, social and leisure activities, education, employment, as well as family dynamics and daily living skills for adults. Additionally, the Case Manager will help with housing and entitlement benefit needs. The primary work schedule is Monday to Friday, although some evenings and weekends may be required.

Community Support
Bridge Health seeks a Community Support professional to work with our APEX Child and Adolescent program. Responsibilities include, but are not limited to, providing recovery, environmental support, and resource coordination services to assist a youth/family in accessing services, building resiliency, and supporting the youth’s emotional and functional growth and development.

Care Coordinator
Bridge Health seeks an individual to join our Child and Adolescent Program as a Care Coordinator. This is a community-based position that requires travel. WINGA, a program of Bridge CSB, has its primary office in Fort Oglethorpe, GA. However, this position will cover the counties listed above. Under general to limited supervision, a Care Coordinator provides facilitation/coordination of care for youth who have been diagnosed as seriously emotionally Disturbed (SED).

Bridge Health seeks an individual to join our Step Up Program as a Certified Peer Specialist (Youth). This position will provide support, advocacy, and linkage to youth and their families. Assists families in interfacing with community providers as part of a collaborative approach to service and/or treatment delivery. Clinical/Mental Health Counselors

Bridge Health seeks qualified clinical/mental health professionals to work in our community outpatient sites. Applicants should have a strong desire to work in a fast-paced environment working with individuals who are in need of mental health and/or substance abuse services.
Under general supervision, provides therapeutic services to adults, children/adolescents, and their families. Services include behavioral health assessments, crisis assessments, individual counseling, family and group counseling, or skills training. This could be in the area of Substance Abuse and/or Mental Health. Maintains clinical records in compliance with all applicable policies and standards.
Monday-Friday 8:30 a.m. to 5:00 p.m. is the primary work schedule. However, we have telework, contract, and part-time positions available in Catoosa, Chattooga, Dade, and Walker County. The work schedule is flexible!
Licensed Practical Nurses (LPN) in our IDD Program (Contract Positions)
Bridge Health is recruiting for Licensed Practical Nurses (LPN) in our IDD Program. These are contracted positions that are primarily based in Chattooga and Walker Counties. Individuals will be responsible for completing Nursing Assessments, and nursing services, and facilitating communication between individuals served, providers, staff, and outside stakeholders such as pharmacies. Certified Peer Support - Parent (Step Up Initiative)
Bridge Health’s Child and Adolescent program is growing!
Bridge Health is recruiting for a Certified Peer Support-Parent (CPS-P): This position covers the counties of Catoosa, Chattooga, Dade, Walker, and Whitfield in Georgia as part of the Step Up program. The Step Up goal is to connect youth and families to services and support and will operate under the System of Care (SOC) umbrella. Certified Peer Support - Parent (WinGeorgia)
Bridge Health is seeking an individual to join our Child and Adolescent Program in the position of Certified Peer Support-Parent. This is a community-based position that requires travel. WINGA, a program of Bridge CSB, has its primary office in Fort Oglethorpe, GA. However, this position will be covering the counties listed above. Under general to limited supervision, a CPS-P will provide support, advocacy, and linkage to the families of youth who are participants of the Wraparound process through WINGA. This position is active in participating in the implementation of Care Plans and Crisis and Safety plans. Performs crisis triage and serves as a liaison to Social Services organizations for the family. CPS-Ps also assist the family in interfacing with DFCS, DJJ, Court Systems, Schools, and other community stakeholders. A CPS-P with WINGA serves as part of a collaboration of care. Maximum work hours will be 9:00a-6:00p, however flexibility of schedule is required as this position supports families within the community. Face-to-face contact with families is required with Parent/Guardian a minimum of one time a week. A billable hour expectation of at least 80% of service time having the ability to be billed is also a requirement. |

Project Light Initiative

Project Light is a program offered by Bridge Health to assist individuals aged 16 to 30 who are experiencing symptoms of psychosis (hearing voices, seeing things, delusions, odd speech or behavior, etc.) for the first time or within the past two years. It is among the agency’s most intensive services, providing frequent contact with a small caseload of individuals to help them understand their diagnosis, provide counseling, case management, and skills training, peer support, education and employment assistance, and medication therapy in order to help them overcome these newfound symptoms and learn to thrive. These services are provided not only in the clinic but in the individuals’ homes and the community. When fully staffed the team consists of a prescriber, a licensed clinician, a bachelor-level paraprofessional, a Certified Peer Specialist, a family specialist, an education/employment specialist, and a part-time Registered Nurse. To be eligible for these services with Bridge Health, an individual must be between 16 and 30 years old, have experienced symptoms of psychosis within the past two years, and live within Bridge Health’s service area.
Board Eligible/Certified Psychiatrist
