  • Care Coordinator for Gordon County and surrounding areas: Under general supervision, performs care coordination for SED children and youth in local school system, DFCS, DJJ, Juvenile Court, and WINGA family homes.   • Call Center Customer Service Representative: Employee will be responsible for verification of patient information and handling incoming calls. Client Support Case Managers needed for TREK Foster Home Program:

Staff will provide oversight for children and the foster homes, coordination of all services needed for children served. Currently recruiting for the locations below.

Contract LPNs needed:

Lookout Mountain is currently recruiting for contract LPNs to provide in-home nursing to IDD individuals. Duties include but not limited to medication administration and nebulizer treatments. State of Georgia licensure required. Schedule is flexible, day and evening shifts are available. Locations are Summerville and Noble.
