Each year, Lookout Mountain Community Services asks all clients to complete a survey to give the agency feedback on their experiences. We would love for you to take a few minutes out of your time to complete the survey! As always, we are focused on providing the best care we can to all of you!
Thank you for taking the time to complete the yearly Client Satisfaction Surveys. Client satisfaction surveys gather positive and negative perceptions that help the agency improve processes and improve the client experience.
- For the individuals who receive services in our Adult programs:

- For the Individuals who receive services in our Kaleidoscope IDD program:
No Initial Appointments are Needed. Walk-Ins are Welcome at an outpatient site between 8:00 am to 1:00 pm.
