WinGeorgia Care Management Entity
WinGeorgia Care Management Entity (CME), a program of Bridge Health, provides Intensive Customized Care Coordination and High-Fidelity Wraparound services for children and youth with complex behavioral health needs. The program helps families coordinate behavioral health, medical, educational, social, and community-based services so youth can remain successful in their homes and communities.
How Wraparound Works
Wraparound is a family-driven, youth-guided, and strengths-based planning process. Each family works with a Care Coordinator to build a Child and Family Team that may include relatives, friends, educators, behavioral health providers, community partners, and others selected by the family.
Together, the team:
- Identifies the youth’s and family’s strengths, needs, and goals.
- Develops an individualized care and safety plan.
- Coordinates services across providers and community organizations.
- Connects the family with formal services and natural community supports.
- Meets regularly to review progress and adjust the plan.
- Prepares the family to maintain progress after services end.
Services are individualized, culturally responsive, and available in English and Spanish.
Referral Considerations
WinGeorgia may serve youth who:
- Have a severe emotional or behavioral health diagnosis.
- Have experienced significant safety or behavioral risks during the past six months, such as self-harm, suicidal or homicidal behavior, running away, fire-setting, property destruction, sexual aggression, or risk of placement outside the home.
- Have experienced an out-of-home placement or are at imminent risk of placement.
- Need coordination among multiple providers, agencies, schools, or community supports.
WinGeorgia cannot accept commercial-insurance-only referrals. Referrals in which the primary concern is a developmental disability, including autism spectrum disorder, or conduct disorder may not meet program criteria.
Final eligibility is determined after the referral and supporting documentation are reviewed.

Complete the Intensive Customized Care Coordination Referral Form and submit it with relevant supporting documentation, such as diagnostic verification, assessments, discharge paperwork, and insurance information.
